How do you refer a patient?
You can refer in several ways. Choose the method that best suits your practice.
Via ZorgDomein
The fastest way to refer. Search for "FemmePrex" in ZorgDomein and fill in the referral form. Your referral is processed immediately.
Via email
Send your referral letter to our secure email address. We process your referral within one working day.

For urgent situations or if you wish direct consultation, you can reach us by phone on weekdays from 09:00 to 17:00.

What information do you need?
To help your patient as quickly and effectively as possible, we ask you to include the following information with the referral.
Required information
- Name, address, date of birth of the patient
- BSN number (citizen service number)
- Health insurance and policy number
- Your AGB code and practice details
- Request for help and reason for referral
Desired additional information
- Relevant medical history
- Current medication (especially hormonal agents)
- Relevant laboratory results
- Previous treatments for menopausal symptoms
- Any contraindications or allergies

For reimbursement by the health insurer, a referral from the GP or occupational doctor is preferred. Patients can also visit without a referral, but then the costs are at their own expense.
Our target is within two weeks of receiving the referral. In urgent cases we try to speed this up. Please contact us by phone in that case.
